Woodbury, NY - Officials with the investigation into the tragic church bus accident that claimed the lives of six people on September 18, 2010, say a cause has been determined, and the bus driver is charged with  felony aggravated unlicensed operation of a vehicle.

According to the report, the New York State Police investigation determined that the cause of the crash on the Thruway in the Town of Woodbury was due to mechanical failure after a tire on the bus blew out. The report also found that the driver, Bernard Lattibeaudiere of the Bronx was driving the bus with a revoked driver's license. Police say the infraction is not the first for Lattibeaudiere--they say the suspect's license has been suspended 25 times on ten different occasions. Latttibeaudiere was arrested last week.

According to a CBS news report, fourteen people were in the van when it crashed on the northbound side of Interstate 87 in Woodbury, about 55 miles north of NYC. The accident, which also injured eight others, occurred at around 3 p.m according to NYS Police.
