An Old Forge man is charged with two felonies after submitting Family Medical Leave Act forms with a forged signature.

State Police say David Roberts, 53, who worked as a New York State Corrections Officer, allegedly submitted the forms to the personnel office with a forged doctor's signature.

Roberts is charged with Criminal Possession of a Forged Instrument and Offering a False Instrument for Filing.

The investigation was initiated by the NYSDOCCS Inspector Generals Office, and investigated in conjunction with State Police.
